Check for sudden load change
Run a Brake Test and make sure both brake circuits are working
Check for a short-circuit in the output circuit
Check the rotation direction(Pr. 359) is set correctly
Check that motor capacity(Pr. 80) is set correctly

Check motor cable
Likely overloaded your brake resistor
